About you

Let's start with the basics

Tell us who you are and how to reach you. This takes about 2 minutes.

Step 1 of 6
Please enter your full name
Please enter a valid email
Please enter your company name
Please select your role
2
Your AI stack

What are you currently running?

Be honest — this is a diagnostic, not a scorecard. Messy answers are the most useful.

Step 2 of 6

Type a tool name and press Enter or comma to add it. Include everything: ChatGPT, Claude, Zapier, n8n, Make, Notion AI, custom GPTs, etc.

0 tools added

Type a workflow and press Enter or comma to add it. Be specific: "lead enrichment via Clay → HubSpot", "invoice parsing with GPT-4", "customer support triage in Intercom", etc.

0 workflows added

Founder / me
A designated ops lead
Shared across the team
External contractor / agency
Nobody formally owns it
💡 Tip: If you're not sure exactly what's running, list what you think is running. Uncertainty here is data — it tells us where the gaps are.
3
Pain & priorities

What's actually bothering you?

The more honest you are here, the more targeted your audit findings will be.

Step 3 of 6

Select everything that resonates. These map directly to the AIGOV dimensions we'll assess.

Things break without warning
I don't fully trust our workflows
Nobody documented anything
We keep adding tools without structure
Unknown AI spend / cost sprawl
Automations overlap or conflict
No visibility into what's working
AI outputs need constant manual review
Scaling before it's properly controlled
Compliance or audit exposure

Be specific. "Our n8n invoice workflow breaks about once a week and nobody knows why" is more useful than "things break sometimes."

Please describe your main pain point (at least 20 characters)
4
Trust & risk

How much do you trust your stack?

These sliders feed directly into the AIGOV drift and risk scoring dimensions. Be honest — the audit is more valuable when you are.

Step 4 of 6

1 = "I check everything manually before acting on it" → 10 = "I trust it completely and rarely verify"

Trust level 5 — Moderate
No trustComplete trust

1 = "Nothing is written down" → 10 = "Everything is documented and current"

Documentation 5 — Partial
Nothing documentedFully documented

1 = "Anyone changes anything at any time" → 10 = "All changes are approved, logged, and reviewed"

Change control 5 — Some process
No processStrict process

What keeps you up at night about your AI stack? Specific incidents, known unknowns, compliance concerns, or anything you've been meaning to investigate.

0 / 800
Please describe any known risks (at least 20 characters)
Automation failure with real impact
AI output was wrong (caught)
AI output was wrong (not caught in time)
Unexpected cost spike
Data or privacy concern
Nothing significant
5
Outcomes & uploads

What do you want from this?

Tell us what success looks like for you — and share any supporting materials you have.

Step 5 of 6

What does a successful audit look like for you? Select everything that applies.

Clear map of current state
Prioritised fix list
Confidence nothing critical is missed
A plan for scaling safely
Something to show my team / board
Compliance / audit documentation
Reduce AI-related cost
Identify what to build next

If there's a specific goal that isn't listed above — a board presentation, a compliance deadline, a team concern — describe it here.

Please describe your goals (at least 10 characters)

Screenshots of your workflows, architecture diagrams, existing docs, Loom recordings, process notes — anything that helps us understand your stack. Max 50MB total, any file type.

Screenshots, workflow diagrams, process docs, Loom links — anything that helps

If you have Loom recordings, Notion docs, Zapier public workflow links, n8n exports, or shared drive folders — paste them here, one per line.

More detail = faster findings. Clients who upload supporting materials receive audit findings on average 1–2 days earlier because we spend less time reconstructing the picture from scratch.

Review & confirm

Everything look right?

Review your responses before submitting. Use the Edit buttons to go back and change anything.

Step 6 of 6

Intake received

Your intake has been submitted. Here's exactly what happens next — and when to expect your audit.

1
Confirmation email — within 10 minutes
You'll receive a copy of your intake responses and a confirmation that we've received everything.
2
Intake review — within 4 business hours
We review your submission and may send one or two targeted follow-up questions if something needs clarification. This is not a delay — it's how we make the findings sharper.
3
AIGOV analysis begins — day 1
Your stack is assessed across all 7 AIGOV dimensions: drift, missing structure, policy tiers, preflight posture, tool policy, cost, and evidence quality.
4
Audit report delivered — day 5 (Starter) / day 7–10 (Deep)
You receive your full written report: current-state map, findings summary, priority matrix, control recommendations, executive summary, and recommended next-step path.
5
Optional review call
You can book a 60-minute walkthrough of your findings. The calendar link will be included in your report delivery email.

Questions? Reply to your confirmation email or reach out directly. Your intake reference will be in the subject line.
